Clinical Pharmacist / Independent Prescriber in Blackpool – ASAP Start

We are looking to recruit an experienced Clinical Pharmacist / Independent Prescriber to join a multi-disciplinary team based in Blackpool.

Responsibilities:

  • Record medication review plans
  • Provide specialist expertise in the use of medicines
  • Be responsible for the care management of patients with chronic diseases
  • Identify and carry out target medication reviews for high risk patients

Requirements:

  • Be GPhC registered
  • Hold a degree in Pharmacy with post graduate experience in pharmacy
  • Independent Prescriber, qualified from approved training pathway or equivalent
